Quoting large groups (100+ employees)

Here's how to request a large group quote for all health plans (including retiree coverage) from Priority Health:

  1. Complete and submit an Agent Agreement, if you haven't already. Download the agreement (600KB PDF).
  2. Have certain information ready when you call. Get a list of what information you need.
  3. Contact your region's sales representatives to request a large group proposal.

Required quoting information for large groups

Basic group & health plan info

We'll need some basic information about the group as well as their current health plan information to create a quote.

General informationCurrent/competitive information
  • Group name (must be the company's legal name)
  • Group locations (must include the group's headquarters)
  • SIC code or type of business
  • Policy effective date
  • Census, which should include each member's:
    • Gender
    • Birth date
    • Tier (single/double/family/waive coverage)
    • Class (active, early retiree, 65+ retiree, COBRA)
    • ZIP code
  • Carrier
  • Current rates and/or renewal rates
  • Current benefits

Health plan history

In addition to what's listed above, we'll need detailed information about the group's specific history based on how it funds its health plan (see table below). Be sure to include your broker commission rates.
