Health management services

Health Management is how Priority Health helps lower employer health care costs and improve employees' health. From worksite wellness classes to help managing chronic conditions, Health Management services combine information, support and coordinated care to keep employees healthy and on the job.

Health Management services are available with any health plan and customizable to meet employers' specific needs. They cover four areas:

• HealthyEncounters - classes and preventive health services
• Utilization and referral management - monitoring the use of diagnostic services and procedures
• Case and disease management - help for people at risk for or suffering from chronic health conditions
• Clinical pharmacy management - making sure employees are taking the best medications for their conditions
