Enrolling a small group

Before you begin...

Keep this important information in mind as you go through the application process:

  • Group applications must have an effective date of the first of the following month and can't be submitted after the 20th of the month.
  • You're required to allow 30 days' lead time for processing.
  • Final rates will be based on final enrollment.

Use ClientManagerSM for all new small group applications. We no longer accept paper applications for new small groups choosing Priority Health coverage.
Required business proofs - for groups with 2 eligible employees only (view the business proofs)
MESC 1017 Quarterly Wage Detail Report - required of all groups, no matter what size

Enrollment forms - use the online form within ClientManager or upload the form into ClientManager as supporting documentation. Download enrollment form (353KB PDF) - Updated 12/2010

Employee Coverage Waiver (18KB PDF)
Only for reform groups of 2-50 eligible employees. Each employee declining coverage must fill out a waiver form.

Enrolling new members during a plan year

When a group needs to add new employees to their coverage, they can use our online Enrollment tool, which employers can access through their online account.

Switching retirees from group coverage to Medicare Advantage plans

Eligible members can take advantage of Priority Health's seamless conversion process for individual Medicare Advantage plans. Learn more.
