Enrolling your clients in individual Priority Health Medicare or Medigap plans

Medicare Advantage and Medicare Part D Drug plans

The annual enrollment period, in which everyone eligible for Medicare chooses their plan for the next coming year, runs from Nov. 15 to Dec. 31. People who don't want to change their plan may do nothing, and their current plan will be renewed automatically.

Enrollment options

You don't need to mail in paper forms. You can enroll your clients using any one of three options:
  1. Online (preferred): Completing and submitting enrollment forms online at prioritymedicare.com is the fastest way to get your forms processed, plus you'll get immediate confirmation that your forms were received. Update any bookmarks or shortcuts on your computer to point to this website.

  2. Print, scan and e-mail: E-mail scanned forms to PH-MedicareEnrollment@priorityhealth.com. Please note that PH-Mailer@priorityhealth.com is no longer accepting e-mail. No need to mail them.

  3. Print, scan and fax: Fax printed, complete forms to 616 942-7204. No need to mail them.

Enrollment tips

Important things to remember when enrolling a client in a Priority Health Medicare Advantage plan:
  • You don't need to mail original enrollment forms if you fax or e-mail them.
  • If you submit an online enrollment form, you'll receive immediate confirmation of receipt. We're working on a notification process for e-mail and fax submissions.
  • Remember to include your agent number on all applications and enrollment forms so we can pay your commission.
  • CMS requires you to complete a Sales Confirmation Form for every one-on-one appointment - download a form (476KB PDF).

Application and enrollment in Priority Health Medigap (Medicare Supplement insurance) plans

Application/enrollment requirements

  • Priority Health Medigap products are underwritten. Individuals can enroll and receive the preferred premium:
    • In their open enrollment period (a six-month period starting the first day of the month after they turn 65 and are enrolled in Part B) or
    • If they have a Guaranteed Issue Right (for example, if they lost or dropped employer coverage, enrolled in a MAPD for the first time and decided to leave the plan in the first 12 months of coverage, or is in a Medicare plan that leaves Medicare)
  • Other applicants may be enrolled at the preferred premium, the non-standard premium, or not at all depending on their health status.
  • Individuals may switch from another carrier's Medigap plan to a Priority Health Medigap plan, but medical underwriting will apply if they aren't in their open enrollment period or doesn't have a Guaranteed Issue Right.
  • Individuals must live in Michigan for at least 7 months of the year.
  • Individuals must be enrolled in Medicare A & B to enroll in a Priority Health Medigap plan.
