Enrolling a large group

Follow these steps to enroll a new large group.

  1. Gather all necessary paperwork from the group:
    • Large group application (243KB PDF)
    • Enrollment form (353KB PDF) from each employee enrolling with Priority Health
    • Binder check for the first month's estimated premium
    • HRA CFS and schedule of reimbursement, if applicable
    • New group exception letter - only if the group application is submitted after the 10th of the month before the requested effective date. Contact your sales representative to request a new group exception letter
  2. Enter all enrollment information into the new group enrollment spreadsheet. You may also email the spreadsheet to the group to have them fill it out and send it back to you. Open the new group enrollment spreadsheet (216KB XLS).

  3. Email the completed enrollment spreadsheet to your representative via secure email.
  4. Submit the group application and all related paperwork to your sales representative.

Enrolling new members during a plan year

When a group needs to add new employees to their coverage, they can use our online Enrollment tool, which employers can access through their online account on this site.

Remember: Make any enrollment changes within 30 days of the qualifying event  (new hires, birth, marriage, etc.).
