This form grants access to the tools your employer groups see online, not bills, reports or other documents. Access these documents through Filemart by calling your sales representative.

Instructions:

  1. Complete this form. All of the asterisked fields are required. You can request persmission for up to 4 users.
  2. Accept the terms and conditions
  3. Click "submit" to send to Priority Health

Once we receive the form, we will forward your request to the employer group to confirm their approval.
