Forgot Password Help

If you've forgotten your priorityhealth.com password, follow these steps:
  1. On any login page (such as the priorityhealth.com home page), click the Forgot Password? link.
  2. Enter your personal Priority Health contract number from your ID card. Include the -00 or -01, etc., if it's on your card. Even if you're a dependent, you have your own contract number.
  3. (Business accounts such as providers and employers only) Enter your username. If you don't remember your username, contact us by phone.
    (Members) Enter your date of birth.
  4. Click the CONTINUE button.
  5. Answer your security verification question or questions.
  6. Enter a new password. Enter it again in the Confirm Password field. (Your old password will no longer work.)
  7. Click the LOGIN button.
  8. You will be automatically logged in to your account at priorityhealth.com.
